Deliver comprehensive home health nursing care while serving as a key liaison among patients, families, physicians, and multidisciplinary teams to improve patient outcomes. This contract role requires expertise in coordinating complex care plans, managing resource utilization, and ensuring continuity of care for diverse patient populations, including those with chronic conditions, frequent emergency visits, and special needs.

Responsibilities:

• Collaborate on the development, implementation, and revision of the case management program to optimize patient care.
• Act as a consultant to healthcare teams regarding patient-specific issues and resource utilization.
• Lead coordination of interdisciplinary care plans, ensuring consistency with clinical progression and covered services.
• Monitor diagnostic testing, treatments, and procedures to guarantee timely and appropriate care.
• Engage with physicians to discuss treatment plans, estimated lengths of stay, and discharge planning.
• Focus on complex cases including chronic pain, substance abuse, frequent emergency department users, and homeless patients.
• Partner with acute care case managers to maintain the appropriateness of ongoing care.
• Facilitate patient transfers to tertiary centers, ensuring transfer of accurate and complete information.
• Oversee data collection to maintain regulatory and accreditation compliance.
• Conduct focused studies on patients with frequent emergency visits for quality improvement.

Qualifications:

• Current Registered Nurse (RN) license with proven experience in home health nursing.
• Strong clinical assessment and case management skills.
• Excellent communication and collaboration abilities within multidisciplinary teams.
• Ability to manage complex patient needs effectively.
• Familiarity with regulatory standards and compliance requirements.
