The Program Manager, Population Health & Clinical Performance is responsible for supporting and leading initiatives that improve clinical and population health outcomes for the Brown & Toland population. This role develops, implements, and evaluates health improvement programs and technical interventions, manages vendor relationships, and oversees complex data analysis and reporting. The Program Manager collaborates with leadership, providers, hospitals, and community organizations to achieve strategic goals, drive quality improvement, and ensure regulatory compliance.

You will focus on:

  • Develop, facilitate, and evaluate population health and clinical performance initiatives.
  • Analyze, review, and trend complex healthcare data; extract data from warehouses and marts.
  • Assess effectiveness and accuracy of new data sources and gathering techniques.
  • Troubleshoot data errors and inconsistencies; coordinate resolutions.
  • Manage vendors and contractors for quality improvement, ensuring contract compliance.
  • Identify issues, trends, and opportunities to improve quality and operational efficiency.
  • Support strategic business activities through data analysis and reporting.
  • Develop and implement business solutions based on research and analysis.
  • Recommend and implement new or modified reporting methods and procedures.
  • Manage large project teams of clinical and technical experts, both internal and external.
  • Track and analyze project performance against plans to ensure ROI.
  • Prepare progress reports for stakeholders regarding project status, barriers, and business intelligence.
  • Define project scope and deliverables in collaboration with leadership and stakeholders.
  • Collaborate with external community organizations to address priority needs and improve patient quality of life.
  • Assist in ACO initiatives, activities, and reporting as requested.
  • Serve as liaison between departments, markets, and data management for reporting needs.
The Skills, Experience & Education You Bring
  • Bachelor's degree in Business Administration, Healthcare Administration, Public Health, Informatics, or related field; or equivalent experience.
  • 5 years' experience in project implementation, product or program management, or healthcare claims/clinical analysis.
  • 3 years' experience leading professional or support staff or in an analytical role using data analysis tools (e.g., SQL).
  • Demonstrated experience with health-related and/or corporate systems.
  • Knowledge and experience with CMS, IHA, and HEDIS clinical quality measure programs.
  • Managed care or prescription benefit management experience preferred.


Base Salary

$89,250 - $107,100/yr

Final determination of a successful candidate's starting pay will vary based on several factors, including, but not limited to education and experience within the job or the industry. The pay scale listed for this position is generally for candidates that meet the specified qualifications and requirements listed on this job description. Additional pay may be determined for those candidates that exceed these specified qualifications and requirements. We provide a competitive compensation package that recognizes your experience, credentials, and education alongside a robust benefits program to meet your needs.

The anticipated pay range for this role is listed in our salary posting for transparency but may vary based on factors including the candidate's qualifications, skills, and experience. This position will also be eligible to participate in our annual bonus program.
