The position involves extensive fieldworkvisiting clients, assessing their living conditions and well-being, and ensuring their safety and access to necessary services. The position also transports and accompanies wards to medical appointments, court appearances, and other essential activities; prepares required reports and documentation; purchases necessary items; explores community resources; and maintains accurate client records using designated software.
