The position is responsible for providing comprehensive intake and ongoing support services to adult wards under guardianship. This includes conducting investigations of the wards residence to locate important personal documents and assets, coordinating with family members, caregivers, and service providers, and assisting with applications for public benefits and identification documents. The position involves extensive fieldworkvisiting clients, assessing their living conditions and well-being, and ensuring their safety and access to necessary services. The position also transports and accompanies wards to medical appointments, court appearances, and other essential activities; prepares required reports and documentation; purchases necessary items; explores community resources; and maintains accurate client records using designated software.
