POSITION SUMMARY:
The Care Enabling Worker assures patients get appropriate and timely services by connecting patients with their Health Care Home and assisting the patient navigate through complex health care and social systems. They work collaboratively with a team of health professionals to provide patients with opportunities to stabilize and improve their health. This position is expected to work within the program directives set by funding sources within the philosophical framework set by the Board of Directors.
EDUCATION/EXPERIENCE:
1. High School Diploma or equivalent education required
2. Associates degree or certification in the Health and Human services field preferred
3. Proficient in the following computer skills: word processing, entering data onto and sorting data in spreadsheets, navigating through the internet, email, and experience with electronic health records.
4. Excellent communication skills (verbal and written) required
5. Must complete annual OSHA training
6. Experience and competency in community outreach work highly desired
7. Prior experience in the Health and Human services setting preferred
