Lead Case Manager

Position Summary

Under the supervision of the Case Management Supervisor and in coordination with the Director of Adult Services, provide team support by coordinating therapeutic interventions with consumers, provide additional service support for the supervisor and case management team. Also shares in the responsibility for the safety and well being of the Hina Mauka consumers. The Team Coordinator will protect the confidentiality of consumers and client records in accordance with all applicable federal regulations.

I. Essential Functions

1. Coordinates with the Clinical Supervisor on therapeutic interventions on high-risk consumers and supports the safety and structure in the milieu and act as the team lead in the absence of the Clinical Supervisor. Also serves as the team leader for other case managers.

2. Evaluates and assess consumers utilizing a bio-psychosocial assessment to determine each consumer's need for services and level of motivation and makes recommendations for appropriate level of care.

3. Coordinates with managed care specialist for approval of care reimbursements; coordinates additional requests for payment authorization as necessary.

4. Based on comprehensive assessment, develops treatment plan in collaboration with each client and the respective treatment team.

5. Assist other case managers to ensure service efficiency and adequate utilization of service units.

6. Communicates and collaborates with referral sources and contract officers to ensure good working and effective relationships.

7. Maintains client records in accordance with Hina Mauka policies and procedures and in compliance with all state and federal laws, licensing and accreditation standards and contract stipulations.

8. Completes billing, fiscal and statistical reporting tasks as required.

9.,Arranges for client transportation for emergency medical care as directed by the Associate Medical Director, R.N.

10. Coordinates staffing and treatment planning conferences as arranged by the supervisor or other stakeholders.

II. Other Functions

1. Performs other related duties as assigned.

III. Qualification Requirements

Education/Experience and Training:

1. Certification as a Substance Abuse Counselor (CSAC) and/or Advance Degree in Social work, Psychology or Behavioral Science preferred.

2. Eligible for certification in CPR and First Aid.

4. Valid driver's license with clean abstract.

Knowledge, Skills and Abilities:

1. Ability to work with and relate to clients with substance abuse disorders, including dually diagnosed and the criminal population.

2. Ability to respond calmly and efficiently to stressful situations with crisis management skills.

3. Well organized and able to maintain accurate and concise records.

4. Able to work with a multidisciplinary team

5. Efficient in Microsoft Programming and familiar with Electronic Health Record systems.

IV. Physical/Mental Demands

Light physical effort (lift/carry up to 10 pounds); occasional standing/walking; occasionally lifts supplies/equipment. May be exposed to infectious and contagious disease. May perform emergency care.

Travel required.
