Are you passionate about mental health care and ready to make a lasting difference in Kona? Join Mental Health Kokua (MHK) as a Behavioral Health Case Manager and support individuals living with psychiatric disabilities on their journey toward stability and community integration.

As a Case Manager, you'll provide trauma-informed, person-centered support to adults navigating serious mental health conditions. You'll coordinate care, monitor progress, and advocate for recovery-ensuring that each client has an opportunity to begin again.

Case Manager Responsibilities:

  • Behavioral Health Assessments: Conduct timely client evaluations to identify needs and determine appropriate referrals for psychiatric and medical support.
  • Service Planning: Create and revise individualized treatment plans; coordinate referrals and monitor goal achievement.
  • Client Advocacy & Coordination: Support clients post-hospitalization and collaborate with providers to reduce re-hospitalization.
  • Insurance & Documentation: Confirm client insurance coverage, maintain accurate clinical records, and complete necessary billing forms.
  • Team Collaboration: Participate in case reviews, supervision, and team meetings to ensure ethical, culturally responsive care.
  • Recovery Support: Empower clients to achieve goals using recovery-focused, strength-based practices.
  • Adaptability: Embrace evolving best practices and maintain program compliance in line with MHK's case management playbook.

Case Manager Requirements:
  • Preferred: Bachelor's degree in psychology, sociology, social work, or a related human/social service field.
  • Preferred: At least 1.5 years of case management or human services experience; experience with dual diagnoses a plus.
  • Required:
    • Valid Hawaii driver's license, vehicle insurance, and current safety inspection.
    • Current TB Clearance, CPR, and First Aid certifications.
    • Must pass a criminal background check and drug screening.
    • Ability to lift up to 25 lbs and travel within Hawaii Island as needed.
