Job Details

Provide direct support to visitors to the Community Connection Center to assist with service and resource navigation, information and referrals, and general assistance Participate in the day-to-day maintenance of the Community Connection Center the ensure a clean and safe environment for visitors Adhere to guidelines and processes of the Community Connection Center and seek appropriate guidance and support from Manager or designee Is knowledgeable of community resources and build positive, productive relationships with community stakeholders to increase supports to members and non-members Participates in relevant and strategic community coalitions, workgroups and taskforces to address SDOH disparities in the community and to enhance the efficacy of UHC services Assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive level of care Utilize both company and community-based resources to establish a safe and ...Health, Worker, Field, Healthcare, Benefits, Patient
