Serves as the engagement lead and primary liaison between Queens Clinical Physician Network (QCIPN) and assigned providers for recruiting, onboarding, and network support activities. - Coordinates and facilitates provider onboarding and ensures that providers understand the support services available to them through QCIPN for clinical care coordination, transitional care management, quality, risk adjustment, and other practice management activities. - Leads the deployment of QCIPN resources to assigned providers based on their needs and investigates and resolves provider operational issues. - Maintains comprehensive knowledge and understanding of various QCIPN contracts and related programs to effectively inform and support providers on key initiatives. - Identifies network growth opportunities and plans and implements activities to engage providers and promote relationship building. - Provides offsite visits to build and strengthen relations with providers. - Executes and drives impl...Communications Specialist, Relations, Clinical, Physician, Network, Communications
