Join our team at Hale Kipa as a full-time Transition Specialist. This role involves providing Cognitive Restructuring groups for youth, helping them learn positive ways to manage thoughts and behaviors to ensure successful community reintegration. The specialist will participate in HYCF treatment team meetings, assist with transition planning pre-release, manage cases to meet service plan goals, and support youth and families in accessing community resources, education, or employment.

Qualifications:

• Bachelor's Degree in Social Work, Psychology, Human Services, Counseling, or related field
• One (1) year of experience working with youth aged 12-21 in skills development, goal setting, and prevention services

Requirements:

• Exceptional verbal and written communication skills
• Proficiency with electronic health record systems
• Timely documentation
• Valid driver's license, auto insurance, and vehicle access for transportation
