Position: Continuum Counselor

Reports to: Clinical Director

Status: Full Time, Part time

Schedule: Flexible

Job Summary

The primary role of the Continuum Counselor is to provide direct substance abuse treatment counseling services, including treatment planning, individualized support, and facilitation across the continuum of services at FTS. The goal is to help ensure positive treatment outcomes and smooth transitions between levels of care.

Qualifications

Master's degree in a behavioral health discipline preferred and/or CSAC required. If no CSAC, a plan to obtain CSAC within one year is necessary. Preference for at least two years' experience in a residential substance use treatment facility and/or supervisory or managerial role. Personal vehicle and valid Hawaii State driver license required.

Essential Functions

  1. Develop clinically appropriate treatment plans with assigned clients and revise as scheduled.
  2. Provide individual substance abuse counseling, group therapy, family counseling, and case management services as assigned.
  3. Conduct classes and psycho-educational groups on addiction, relapse prevention, family issues, women's issues, and other relevant topics.
  4. Maintain accurate and current clinical records for each resident, noting significant problems and progress using the WITS system.
  5. Prepare written reports and correspondence related to case management professionally and promptly.
  6. Communicate ongoing updates to program staff or referral agencies to ensure coordinated services.
  7. Participate in treatment teams across the continuum of services at FTS.
  8. Work closely with the Clinical Supervisor to support positive structures, policies, procedures, and training.
  9. Stay updated on agency training and professional development requirements.
  10. Keep the work area neat, orderly, and sanitary.
  11. Follow agency procedures promptly for handling Emergency Response Codes.
  12. Maintain all necessary credentials and certifications as per agency policy.

  • Perform other related management duties as assigned.

Other Requirements

Able to meet basic physical, mental health, and communication standards as per State and professional accreditation standards. Compliance with background checks, physical exams, CPR/First Aid, TB testing, and driver record checks is required.
