Summary

Provides case management and career development services for participants in self-sufficiency programs. Provides group and individual educational and vocational guidance services in line with the most current job trends and labor market needs to best assist participants in finding employment of their choice. Provides career counseling, job placement, and job counseling/retention and advancement after job obtainment.

Provides career counseling, job placement, and job counseling/retention and advancement after job obtainment.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

Collects, organizes and analyzes information about individuals through records, tests, interviews and professional sources to appraise participants' interests, aptitudes, abilities and personality characteristics for vocational planning and/or employment placements.

Compiles and studies occupational, educational and economic information to aid participants in making and carrying out vocational and educational objectives.

Assists individuals to reduce social and emotional barriers.

Provides employment and case management services to participants that are comprehensive and person-centered, which include but are not limited to:

  • Tracking participants' progress
  • Assisting in setting goals; creates Employment Plan
  • Recording participant contact (case noting)
  • Building and sustaining rapport with participants
  • Maintaining documentation for participant case records in accordance with the funder's policies and procedures
  • Collects, organizes and analyzes information about individuals through records, tests, interviews and professional sources to appraise participant's interests, aptitudes, abilities and personality characteristics
  • Inputs data into State computer system on participant activities; working knowledge of computers for data entry – state client accountability system to include ECF (DHS funded work programs)
  • Generates payments for supportive services: childcare, transportation, and other work-related expenses.
  • Communicates effectively with State Agencies (state employees and other program offices) verbally and in writing
  • Educates about and refers participants to resources, opportunities, and available supports
  • Completes transfers and case closures in accordance with State standards and timelines

Selects or develops teaching aids such as handbooks, demonstration models, multimedia visual aids, reference works, and computer tutorials that support individualized training, group instruction and workshops

Conducts orientation as well as preliminary and follow-up interviews with participants.

Able to utilize and provide guidance to interpreters regarding expectations and needs for interpretation during workshops and trainings.

Collaborates with clients in developing an Individualized Service Plan (ISP) and establishing employment and career goals.

Responsible for gathering data from caseload on a monthly, quarterly, and as needed basis as it relates to agency and program policy and procedure for reporting purposes. Verifies that data is correct and submits reports to supervisor timely.

Ability to deal effectively with unresponsive, uncooperative, difficult, and/or on occasion, irate participants. Demonstrate ability to deescalate individuals with tact and understanding.

Uses Motivational Interviewing techniques to assist participants to understand and reduce social, motivational, emotional, and other problems which could constitute a barrier to employment. Refers participants to outside resources as needed.

Attends and participates in meetings, staff development sessions and in-service training as assigned.

Assists in the preparation of Branch Reports for the Fair Hearing Process.

Assists with participant, employer and funder satisfaction surveys on a quarterly basis.

Manages all program operations in accordance with agency policies, contract, and State and Federal law including HIPAA guidelines.

Supports and promotes all Ohana Nui initiatives.

Responsible to understand and support CARF accreditation standards.

Works on special projects as assigned by the Program Coordinator.

Conducts work in a safe and responsible manner, utilizing protective clothing and/or equipment when appropriate. Complies with all safety standards. Responsible for being aware of and adhering to all company and safety policies and procedures.

Supervisory Responsibilities

There are no direct supervisory responsibilities.

EDUCATION And/or EXPERIENCE

High school diploma or equivalency with no less than one to two years related experience in social work, human services, counseling, assessments, goal planning, job development, customer service or conflict resolution or equivalent combination of experience and education.

Language Skills

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.

Mathematical Skills

Basic math skills such as add, subtract, multiply, and divide.

REASONING ABILITY

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

CERTIFICATES, LICENSES, REGISTRATIONS

CPR Certification may be required as delegated by management.
