Are you an experienced behavioral health professional living on Oahu and ready to step into a leadership role in case management? Mental Health Kokua (MHK) is seeking a compassionate and skilled Case Management Team Leader to guide, support, and inspire our team of Case Managers while upholding the highest standards of care for individuals living with serious mental illness.
In this role, you'll maintain a small caseload while providing clinical oversight and supervision-ensuring coordinated services, regulatory compliance, and a person-centered approach that empowers clients on their journey toward recovery and community reintegration.
Case Management Team Leader Responsibilities:
• Caseload & Clinical Support: Maintain a caseload of 10-15 consumers, providing direct support, treatment planning, referrals, and coordination of psychiatric and medical care.
• Staff Supervision: Provide weekly supervision to Case Managers, reviewing consumer trackers, treatment plans, acuity levels, and hospitalization follow-ups.
• Treatment Oversight: Assist staff with behavioral assessments, care coordination, medication management, and individualized service delivery.
• Documentation & Compliance: Monitor and update electronic health records; ensure all services comply with MHK policies and regulatory standards.
• Collaborative Care: Facilitate case conferencing, attend MED management and quality improvement meetings, and coordinate with interdisciplinary teams.
• Client Advocacy: Support access to entitlements, housing, and community resources. Provide crisis intervention and promote consumer independence.
• Quality Improvement: Participate in MHK's Quality Improvement Committee and contribute to ongoing program evaluation and service enhancement.
• On-Call Coverage: Provide after-hours support as assigned, including evenings, weekends, and holidays.
Case Management Team Leader Requirements:
• Bachelor's degree in psychology, sociology, social work, human services, or related behavioral health field (required); Master's degree (preferred).
• Three (3) years of experience in case management or human services, with prior supervisory experience strongly preferred.
• Knowledge of mental health, dual-diagnosis, community resources, and psychosocial rehabilitation principles.
• Strong leadership, communication, and problem-solving skills with a commitment to team-based, client-centered care.
• Proficiency in electronic documentation systems and ability to maintain timely and accurate records.
• Must pass a criminal background check and maintain current CPR, First Aid, TB clearance, and a valid Hawaii driver's license with clean abstract.
• Ability to lift up to 25 lbs and travel between program locations.
