AssessmentProvide telephonic screening of BH members using PHQ-9, GAD, and other CM-DM diagnostic tools to determine additional care support needs.CommunicationCommunicates with HMSA members, physicians, case managers, and disease management team members.Coordinate with other colleagues to ensure continuity of care for members in assigned work queue.TriageAfter assessment, triage members to appropriate BH programData Management and DocumentationComplete and file forms and input data into appropriate HMSA systems.EducationProvide navigation, referrals to community resources, and educational materials.Performs all other miscellaneous responsibilities and duties as assigned or directed#LI-HybridBachelor's degree in social services and one year of work experience conducting assessments in behavioral healthEffective verbal and written communication skillsStrong organizational skills.Knowledge of the appropriate protocol to be followed for a given BH diagnosisBasic working knowledge of Microsoft Office applications. Including but not limited to Word, Excel, and Outlook.Reliable transportation to attend in person (Face to Face) meetings
